I played FFIV DS I casted Reflect on my party to Reflect Bahamut's Mega Flare. I then realised you can't reflect mega flare which then took out all of my party ;_;

In Kingdom Heart II, I just recently tried fighting Demyx on Proud Mode. It didn't go so well, with me attempting 5 times, and sometimes dying at the beginning of the fight. >_>

Playing FF8 through again. Being slightly experienced at the game, I've managed to amass epic magic masses very early on, hitting stuff for a nafty 5000 damage with Darkside, having about 3000hp and essentially one-hitting every boss. Well, Edea at the end of disc 1 required three. :<
Anyway, enter disc 2. Dream sequence whereas I run around as Laguna whacking meaningless fodder enemies. Stuff that has some 500hp. Laughable. I'm just waltzing through them, until one of the enemies, a pitiful caterpillar uses some ability by the name of Sticky Web. Does no damage, and, from my knowledge at that point, has a 50/50 chance of doing nothing or casting Slow on me. This time, however, it casted Stop. My jaw just dropped as I stare on in disbelief as my invincible Laguna stands there like a statue, incapable of doing a thing as the miserable weaklings are sluggishly, oh so sluggishly, doing some 80damage per hit as they gnaw down the hp I had left. I believe Stop should fade off after a little while. Well, in this case, it didn't. I died. The amount of humiliation was unspeakable.

better late than never then? My most Frustrating would be Ninja Gaiden Sigma on normal mode, I got frustrated at my freakishly low health bar, so I decided to do easy mode, now its kinda fun... also there was the Luiqid vs Snake battle on top of metal gear Rex in MGS 1, Those head buts annoyed the crap out of me! aswell as Killzone 2 did against the last stage of wave after wave of enemies, then Visarii's Cheap shanks... also, in assasins creed when you would do a mission, there would be a long "cutscene" that's unskipabble, then after failing it like 8 times they would get annoying as hell! and then there was the Resistance levels where You got to one check point, got pwned! and had to go ALL the way back to the Checkpoint Disgaea 3's Grinding, to fight a boos that defeats you again, so more grinding for you... Valkyrya Chronicles long battles, that you ended up loosing, so you had to save in bewteen battle... LBP physics engine

For myself...
DMC3's - Dante Must Die mode. (Which is kinda funny because DMC3 is supposedly harder than DMC1, but for em it feels like DMC3 is actually a bit easier, even on Hard. )
Anyway, I got to Angi & Rudra. (A boss fight which annoyed me even on normal and hard.) Which makes this exceptionally tough for me. I typically get one 75% dead and the other 50% dead then the one I'm not focusing on gets me. (It's much easier to bring them both to like 99% dead then hit them both at same time to end the fight then to focus entirely on one and kill it and have the other get both swords and do combo moves on me...)
Call of Duty 4: Modern Warfare (when I played Online last, which was in September I think.) - I always seem to get matched up against teams of clanners. And they always snipe me from everywhere. Seriously, it seems like I breathe and they snipe me with a pistol from the other side of the map in the fog/rain through 2 buildings.
Other than that, not too many games I play do I easy get beaten/lose. (And I normally play on Normal first play through then if I do another play through I oft go the next harder difficulty.)

Hito-Shura in Digital Devil Saga. God that battle is completely insane. You need extreme stats (And Level 99 of course) and luck to beat this beast. (Then again, I would expect no less from a battle with Hito-Shura after all the things he managed to do in Shin Megami Tensei 3: Nocturne) www.youtube.com/watch?v=ynzL2TnEDec&feature=related&fmt=18Penance in FFX. Read above. It's pretty much the same except this one takes one hour to beat even if you're dealing 99999 damage per turn with Haste. www.youtube.com/watch?v=NVYjVrEgCkA&fmt=18EDIT: Added some links of people beating them
